Health Improvement Team

We work together with healthcare providers, residents, community based organizations and city leaders to assess health status, determine priorities, build action plans and provide leadership around health issues that impact the community.

The Health Improvement Team (HIT) is a consistent presence in these communities and brings knowledge to improve both patient care and community health. This work is centered on addressing the root causes of health inequities. Through this structural lens, our team brings partners together to identify social and environmental factors that influence health and works to build equitable programs, policies and systems that support every person to reach their best health.
