In January 2022, many CHA primary care providers became part of an innovative Medicare program called Primary Care First. This program was designed to make sure your CHA provider has all the information they need to make sure yourhealth care is coordinated across all of your providers.
Sharing Your Information
Medicare makes sure that CHA providers get information from other specialists, emergency departments or hospitals. That way your CHA provider will be able to understand your health needs and will be able to better coordinate your care.
Medicare will not share health information related to substance use treatment unless you give them permission to do so.
Your Medicare Benefits
As a reminder, this program will not change your Medicare benefits. Although we encourage our patients to get care at CHA or within our preferred network, Medicare beneficiaries always have the right to visit any doctor or hospital that accepts Medicare.
