CHA Union Square Family Health has what you need. Our primary care experts see people of all ages to help you, and your family, live your healthiest lives. Our services also include Women's Health and Pregnancy care.

As the first Patient-Centered Medical Home at CHA, we are as a national model for team-based care. This means when you join us, you get a whole team that works together to keep you healthy.

We are proud to care for entire families across as many as four generations from the greater Boston area.

    Parking:

    Our main entrance is on Revolution Drive in Somerville, across from Home Depot. We have a free parking lot next to the building, though spaces are limited at peak hours.

    If you are using GPS to get here, please read this important note before you visit us.

    Public Transportation:

    Bus: 9095

    Notes: From Sullivan Square, take the 95 bus (please check with driver to make sure the bus is going to Assembly Square). Get off at the stop after Home Depot. If you take the 95 bus from Medford, it lets you off on the wrong side of the highway. You have to go to Sullivan and take the 95 outbound.

    Train: Orange Line, Sullivan Station, Assembly Station.

    Notes: If you are walking from Assembly Station, go past the Partners building and cross Grand Union Boulevard. Home Depot will be on your left and our parking lot on your right. CHA is in the large grey and blue building on your right after you pass the Bank of America ATM.

  • Using eCheck-in Before Your Visit

    If you have a MyCHArt account, you can now use eCheck-in to review and update your information before you come for an in-person visit.

    Your visit will be safer, and you can save time at the registration window when you arrive!

  • COVID Guidelines
    Who can come with you or visit you in the hospital?

    Patients coming for a clinic visit, to Urgent care or to the Emergency Department are allowed to bring at least one person with them. Additional people will be at the discretion of the provider and clinic.

    Your primary care or specialty team knows when you are due for routine tests, immunizations and health screenings.

  • Get refills?

    For the best service, just call your pharmacy to get your prescription refilled.

    Please don’t wait until your medication is gone before calling. However, if you do run out, please tell the pharmacist. They may be able to give you some medication until your refill has been approved.

  • Get help when I feel sick?

    Same day virtual visits when you feel sick.

    You can schedule a same-day or next-day Virtual Sick Visit through MyChart for certain symptoms. Just log into MyChart and click the Visits button. This will let you schedule a video appointment for things like cough, headache, COVID-19, back pain, vaginal itch or discharge, painful urination, and more. If you have a symptom not included on the self-booking list, please call your primary care office. You’ll be connected to our Nurse Advice Center who can help you get the care you need.

    At the time of your video appointment, you’ll get a link by text and email. Just click on the link and log in using the camera on your phone or computer. Depending on your needs, we can provide medical advice, prescribe medicines, and set up a follow-up visit. Get the medical care you need from CHA without having to leave your home! 

    If you have an urgent medical need (not an emergency):

    Please visit CHA Somerville Campus Urgent Care at 33 Tower Street, Somerville.

    If you are having a health emergency:

    Call 9-1-1 or go to the nearest emergency room.

    You can find 24 hour care with short wait times at CHA Cambridge Hospital and CHA Everett Hospital. When you go to a CHA emergency room, your CHA doctor will be notified about your health needs and can help coordinate your care.
